Your Health Concerns & Health Goals

Lots of things are discussed and happen at a doctor’s visit. Making and bringing a list can help you remember everything you wanted to ask and discuss with your doctor.

Your Medical & Surgical History

If possible, have a list of your medical conditions, prior surgeries, and recent lab results. 

Or, have the name and contact info of your previous doctors so we can request your medical records. 

Your Family Medical History

It can be helpful to ask your family about their medical history beforehand 

Your Current Medication List

Bring your medication bottles or a list that includes the dosage and frequency

Your Health Screenings

Your doctor will want to check and discuss your preventative health screening tests to keep you up-to-date. If possible, bring your recent colonoscopy (or other colon cancer screening tests), women’s health screening results (such as mammogram, pap smear), vaccine records. 

Your Health Team Members

Many people have specialists taking care of them. It’s helpful to know who else is in your healthcare team.
